Liability Waiver and Media Release

I, the above named individual, hereby certify that I am over the age of 18 and voluntarily agree to participate in the Queen City Line Dance Classic event. I acknowledge and understand that participation in this event, including but not limited to dance lessons and social dancing, involves inherent risks of physical injury. I assume full responsibility for any injury, loss, or damage that may occur as a result of my participation.

I agree to release, indemnify, and hold harmless the event organizers, owners or lessors of the premises, and their agents, employees, and affiliates from any and all claims, liabilities, demands, or causes of action arising out of or related to my participation in this event, whether caused by negligence or otherwise.

Furthermore, I grant full permission to the event organizers and their representatives to record, reproduce, distribute, broadcast, and publicly display any photographs, videos, or other media captured during the event for promotional, commercial, or any other lawful purposes, without limitation or compensation.

I acknowledge that I have read and fully understand the terms of this waiver and release, and I voluntarily sign this document as a condition of my participation.

Event Registration Policy  Event registrations is non-refundable. In the case of an extreme circumstance, please contact the event directors for reconsideration or possible rollover to the following year.

Shirt and book fees are 100% refundable up to two (2) months prior to the event start date.
